HOW TO GET A VACCINE
If you are 18 or older, and are an established patient* of La Pine Community Health Center, please call us at 541-536-3435 to request your COVID-19 vaccine.

MEDICAL EXEMPTIONS
The following information applies to individuals who are seeking medical exemption from the COVID-19 vaccines and need an exemption form completed by their provider for their employer, school, etc.:
Our priority is to protect you, our staff, and all members of our local community from getting severely ill from COVID-19. For this reason, our primary care providers (PCP) will not be granting any medical exemptions from the COVID-19 vaccines EXCEPT for the very rare case of clearly documented severe allergic reaction to a prior vaccine. The proof of the severe allergic reaction must include a doctor visit or an ER visit when such a reaction occurred with clear documentation that the allergic reaction was due to the vaccine itself. The reaction also must show that there was concern of throat closure or swelling.
Your PCP may also grant you TEMPORARY exemption from the COVID-19 vaccine if you are currently positive for COVID-19 and are symptomatic. This exemption will only be granted until you have completed a 10-day quarantine period and are symptom free.
These would be the ONLY cases in which your primary care provider may grant a medical exemption for you. All other requests will be denied.

DO NOT DELAY CARE
Delaying care for chronic illnesses (diabetes, heart disease, lung disease, etc.) or new medical concerns as well as avoiding preventative healthcare could drastically increase your chance of being infected with COVID-19. It is important that you stay on top of these illnesses so that you can protect your immune system and reduce your risk of needing to visit the emergency department or be admitted to the hospital.
